Passport Application Checklist
Arrive 15 minutes prior to your appointment time. Applicants are subject to screening and may not bring in electronics.
You must bring the following or you may have to reschedule your appointment:
1) Passport application form
Complete the form at https://pptform.state.gov/ . Print the completed form single sided (make sure that it has a barcode on the top left side). Do not sign the form until asked to do so at the appointment. All passport applications must include the applicant’s Social Security number.

- If you do not remember your Social Security number, contact the Federal Benefits Unit.
- If you were never issued a Social Security number, enter all zeros in the application form’s Social Security number field. If this is your case, you must complete and sign a statement to certify that you have never been issued a Social Security Number and bring it to your appointment.
2) Passport Photo
Your 5 x 5 cm U.S.-style passport photo. The photo must have been taken within six months of your application date. The photo must be clear and taken in front of a white background. Glasses may not be worn in the photo.
3) Printed Copy of Payment Confirmation

- Bring a printed copy of your payment confirmation receipt to your appointment.
- Use the Fee Calculator to calculate the cost of your new passport.
- Pay the passport fee online. Check the bottom left corner of the application form to determine whether it is a DS-11 or DS-82 form. Click here to pay the DS-82 fee , or here to pay the DS-11 fee .
- Please schedule your appointment at least 48 hours after payment submission. This provides time for your payment to be processed prior to your passport appointment. If your payment has not cleared by the time of your appointment, we will not be able to process your application and you will need to return the Embassy or Consulate in your district at a later date once your payment has cleared.
4) Supporting Documents
Bring the original and one photocopy of the following documents:

- Proof of U.S. citizenship: A valid or expired U.S. passport book and/or card, a U.S. birth certificate, Consular Report of Birth Abroad, or U.S. naturalization or citizenship certificate. Click here for examples of citizenship evidence.

5) Additional Requirements for Minors
Under 16 Years of Age

- Original and one photocopy of the applicant’s birth certificate. If the applicant was born in Turkey, it is mandatory to bring the Turkish birth certificate along with the Consular Report of Birth Abroad, the Naturalization Certificate or the Certificate of Citizenship.
- Must be accompanied by all legal parents with their valid ID and a photocopy of each ID. If all legal parents cannot be present, an original notarized DS-3053 form for the absent parent must be submitted with the application. 16 or 17 Years of Age

- Original and one photocopy of the applicant’s birth certificate. If the applicant was born in Turkiye, it is mandatory to bring in the Turkish Birth Certificate along with the Consular Report of Birth Abroad, the Naturalization Certificate or the Certificate of Citizenship.
